     Take a few minutes to reflect on how you have been feeling over the last six months; then check the boxes that apply to your situation.


Anxiety

Anger

Think of ways to “get even.”

Alcohol Misuse
When I drink, I have a tendency to say or do things I normally wouldn’t do when sober.
I neglect my family or obligations when drinking.
I tend to use alcohol as a main source of relaxation.
I find I do not remember part of the evening.
I feel guilty or experience a sense of remorse when I drink.
